Dr. Jeffrey Forman, former chair of Radiation Oncology, dies at 63

Jeffrey Forman, M.D., FACR, medical director of Development for the Barbara Ann Karmanos Cancer Institute and former chair of the Wayne State University Department of Radiation Oncology, died April 8. He was 63.

During his 14-year career with the institute, he served as medical director of affiliate programs and satellites, medical director of philanthropy, chief and chair of the Multidisciplinary Urology Oncology Program, director of the Neutron Therapy Program and associate director of Clinical Care.

Named a Health Care Hero in 2004 by Crain’s Detroit Business, Dr. Forman became a professor of Radiation Oncology in 2007. He left WSU and Karmanos in 2007 to serve as medical director and national director of prostate cancer research for 21st Century Oncology in Michigan. He retired from practice in 2017 to become medical director of the Jewish Hospice and Chaplaincy Network.

He returned to Karmanos to serve as medical director of Development in May 2018.

Dr. Forman was a tireless proponent for patients and for the cancer institute.

He is survived by his wife, Miriam Forman; sons Dr. Adam (Marianna) Forman and Joshua (Norah Rexer) Forman; daughter, Tara (Jay) Hack; mother, Eileen Forman; brother Randee (Marvin) Tancer; sister-in-law Marilyn (Dr. Ira) Bernstein; and brother-in-law Steven (Maureen) David.

The funeral will be held during a private graveside service at 12:30 p.m. April 12, with the Rabbi Paul Yedwab officiating. Click here to watch the live video of the funeral service.
